Diabetes Management
The goal of the Diabetes Management Programme is to ultimately reduce a patient’s hemoglobin A1C level.
Our nurses, working with the patient’s physician, provide support, education and assistance to diabetic patients and their families.
Our diabetes program combines at home diabetes care, complemented by support through a diabetes management portal designed to specifically help patients manage and monitor their condition.
